St. Tammany Health System is seeking a Care Coordinator RN to join our team in Covington, LA.
The Care Coordinator assesses, plans, implements, coordinates, and evaluates options to meet a patient’s health needs, ensuring a patient-centered, high-quality, evidence-based plan of care that improves access and outcomes. Ideal candidates hold RN licensure, 3–5 years of hospital adult acute care nursing experience, BLS, and CCM/CMAC/ACM preferred certifications.

Scheduled Weekly Hours: TBD. Day shift - weekday and weekend shifts.
Care Coordination is a collaborative process in which the Care Coordinator assesses, plans, implements, coordinates, and evaluates options and services to meet an individual's health needs. The Care Coordinator facilitates progression of care and monitors the patient's progress to ensure that the plan of care and services provided are patient focused, high quality, evidence based, appropriate to patient needs, efficient, and cost effective. Care Coordination improves access to services to improves outcomes and reduce over-utilization of resources.
Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 10 pounds is required. Must be able to work with a moderate level of noise. Physical Effort required: Constant (67%-100%) - talking, hearing, seeing Frequently (34%-66%) - NONE Occasionally (1%-33%) - lifting, carrying, pushing/pulling, climbing (stairs, ladders, etc.), balancing, stooping, crouching, reaching, handling/feeling
