Your Health Organization is seeking a Registered Nurse to join our growing urgent care team. This role focuses on service across South Carolina and Georgia, coordinating care with providers, facilities, homes, and clinics to ensure seamless patient management.

Ideal candidates are licensed nurses with 3+ years of clinical experience, strong leadership, and excellent communication. The position involves collaborating with hospice teams, conducting telehealth visits, and maintaining accurate EMR

Qualifications

• Must be a licensed nurse and in good standing with the board.
• Minimum of three (3) years clinical experience preferred.
• Ability to supervise and direct professional and administrative personnel.
• Ability to read and communicate effectively.
• Ability to lead and demonstrate strong leadership skills.
• Strong interpersonal and communication skills in all conditions.

Responsabilités

• Admissions and patient intake coordination.
• Coordinate with care teams for comprehensive patient plans.
• Facilitate telehealth visits with nurse practitioners for care coordination.
• Collaborate with hospice interdisciplinary team and care plan providers.
• Maintain and update EMR with current patient information.
• Communicate with patients, families, and caregivers; participate in coaching calls.
