Job summary

Your Health Organization in South Carolina is seeking a Registered Nurse to join our urgent care team, providing care coordination for patients in homes, assisted living facilities, clinics, and via telehealth as needed.

Qualifications

• Must be a licensed nurse with license in good standing.
• Minimum of three years clinical experience preferred.
• Ability to supervise and direct professional and administrative personnel.
• Ability to read and communicate effectively.
• Ability to manage and demonstrate effective leadership skills.
• Excellent interpersonal and communication skills under all conditions.

Responsibilities

• Admission coordination and care planning.
• Communicate, collaborate, and coordinate care.
• Continuous review and revision of CCM plan of care.
• Facilitate telehealth visits with the nurse practitioner for coordination of care.
• Coordinate with hospice interdisciplinary team and other care plan members.
• Participate in hospice IDG meetings.
• Coordinate with Assisted Living and Skilled Nursing Facilities.
• Update of patient care plans and care team assignments.
• Maintain current provider and specialist lists in the EMR and communicate with patients and families.
