Job summary

Corewell Health in Wayne, MI seeks a Part-time Care Manager / Utilization Review RN to coordinate admission, discharge planning, and care transitions. You will work with physicians, payers, and the inter-disciplinary team to ensure appropriate care levels and cost-effective outcomes.

The ideal candidate has a Michigan RN license, BSN preferred, 2+ years in acute care and 3–5 years in care management, utilization review, or discharge planning; BLS preferred.

Qualifications

• Bachelor's degree from an accredited nursing program.
• Active Michigan RN license upon hire.
• Minimum 2 years of experience in acute care.
• 3–5 years of experience in care management, utilization review, home care and/or discharge planning (preferred).
• BLS certification preferred (AHA or ARC).
• Non-BSN candidates pursuing BSN within 2 years may be considered.

Responsibilities

• Identify patients needing care management services (utilization review, care coordination, discharge planning).
• Manage a caseload to facilitate admission, stay, and discharge planning.
• Collaborate with care team, payers, and physicians to optimize patient flow and resources.
• Use criteria like InterQual to determine medical necessity when appropriate.
• Communicate with payers, patients, families, and post-acute providers to coordinate transitions.
