Job summary

UW Health is seeking a Registered Nurse (RN) for the Home-Based Primary Care program in Madison, WI. This 100% FTE role focuses on care coordination, triage, and case management for homebound adults, with collaboration across MDs, APPs, social workers, schedulers, and RN Care Coordination nurses.

Shifts are Monday–Friday, 7:30 am–5:30 pm, with 8- or 9-hour days and no weekends. Onsitework at UW Health facilities in Madison.

Qualifications

• BSN preferred or higher.
• Experience coordinating care for adults—geriatric population a plus.
• Strong communication and triage skills required.

Responsibilities

• Coordinate care for Home-Based Primary Care patients via phone or video.
• Triage emergent issues and facilitate referrals and care plans.
• Collaborate with MDs, APPs, Social Workers, and other nurses to support patient care.

100% FTE. The scheduled shifts are Monday-Friday, 8 or 9-hour day shifts (10 hour shifts not available) between 7:30 am to 5:30 pm. No weekends. When onsite, you will work at the Administrative Office Building on UW Health Court, or the Northport Dr. Clinic, both located in Madison, WI.

Pay:

Join the innovative Home-Based Primary Care program working with the adult/geriatric population that are homebound.

We are seeking a Registered Nurse (RN) to:

• Collaborate with a focused interdisciplinary team of MDs, APPs, Social Workers, Schedulers and other RN Care Coordination (RNCC) nurses to care for patients enrolled in the program.
• Perform the main duties of the RNCC nurse that include care coordination, triage and case management. Communication with patients/families is via telephone or video communication (no in-home visits at this time).
• Utilize your nursing skills such as triaging emergent issues, providing resources to clients and their families, initiate/facilitate consults/referrals, update care plans, and work with rehab/LTC facilities to coordinate care. It is preferred that nurses have 3-5 years of adult acute care or primary care nursing experience.

Qualifications

• Graduate of school of nursing Required
• Bachelor's Degree in Nursing or higher Preferred

Work Experience

• 6 months RN experience Required
• Previous experience in adult acute care or ambulatory setting Preferred
• Previous Case management experience Preferred

Licenses & Certifications

• Registration as a registered nurse in state where employed or licensure in the state licensure compact Upon Hire Required
• CPR/BLS Upon Hire Required
• Case Management Certification Preferred
