Job summary

Qualifications

Responsibilities

• Coordinate discharge planning with interdisciplinary team and external providers.
• Document care plans and updates in the EMR.
• Educate patients and families about care options and resources.
• Collaborate with physicians, nurses, social workers and other staff.
