Job summary

Qualifications

• AS in Nursing; must be actively pursuing Bachelor's Degree; Bachelor's Degree completion required within 3 years of hire.
• Nurse Licensure Compact (NLC) accepted in lieu of Kentucky Nursing License per NCSBN rules.

Responsibilities

• Promotes optimal management of clinical resources by conducting timely initial admission assessments and coordinating care.
• Coordinate discharge planning with internal and external providers; maintain patient confidentiality.
• Educate the healthcare team about the RN: Case Manager role and serve as a resource to physicians, nurses, and staff.
• Ensure documentation is complete in the EMR and complies with HIPAA requirements.
