Description:We are an outpatient day program that provides education and exercise programs to cardiac and pulmonary patients.We offer a multidisciplinary program with registered nurses, registered dietitians, exercise physiologists, and physical therapists.Patient groups are assigned two professionals with a clinician to patient ratio of 1:5.The patient population includes adults aged 25 to 90 years.Cardiac rehab sees patients with recent cardiovascular events or diagnoses, such as myocardial infarction, stent placement, CABG, valve repair/replacement, transplant, heart failure, and stable angina.Pulmonary rehab addresses patients with COPD, post-COVID conditions, and other lung diseases.Census varies based on group attendance.Charting is conducted using an electronic medical record system and a separate telemetry system.Floating is not required.Rotating shifts are not required as we operate a day program from 6:45 AM to 5:30 PM.
