Care Initiatives in Waterloo, IA is seeking a Clinical Program Director (RN) to lead the Cardio-Pulmonary Program within a 94-bed skilled nursing center. You will be the clinical expert, partnering with physicians, hospitals, residents, families and interdisciplinary teams to drive outcomes and deliver resident-centered care.

You will develop education programs, supervise rounds and transitions, and lead program growth while advancing evidence-based practice and continuous improvement in care

Qualifications

• Current, unrestricted RN license in Iowa.
• Clinical experience in cardiac/pulmonary or related care.
• Strong assessment, critical-thinking, and problem-solving skills.
• Ability to educate, coach, and mentor healthcare professionals.
• Excellent verbal and written communication skills.
• Collaborate with physicians, hospitals, residents, families, and teams.
• Strong organizational and time-management abilities.
• Commitment to resident-centered care and outcomes.

Responsibilities

• Serve as the clinical leader and SME for the Cardio-Pulmonary Program.
• Support nursing staff in changes in residents' cardiac and respiratory status.
• Participate in rounds and resident reviews to optimize care plans.
• Collaborate with physicians and interdisciplinary teams for evidence-based care.
• Monitor outcomes and recommend improvements.
• Develop and deliver cardio-pulmonary education programs.
• Lead onboarding and specialty orientation for program staff.
• Conduct competency validation and ongoing education initiatives.
• Educate residents and families about cardiac/pulmonary conditions and care.
• Support transitions home or to other care levels.
• Assist with hospital-to-SNF transitions and discharge planning.
• Promote continuity of care with partners.
• Participate in QA and performance improvement initiatives.
• Occasional travel to other locations as needed.
