Responsibilities

• Provides high-quality, patient-centered clinical care.
• Performs initial and ongoing patient assessments.
• Documents assessment and communicates with the interdisciplinary team.
• Develops and documents individualized care plans and services.
• Complies with safety, medication, and documentation policies.
• Revises care plans based on ongoing evaluations; maintains BLS.
