Firelands-Health in Ohio is seeking an experienced Registered Nurse to provide patient-centered care on a dedicated caseload. The RN will update treatment plans when medications are used and collaborate with the behavioral health team to support patients with chronic conditions.
The role requires an Ohio nursing license and at least 3 years of healthcare experience. Telehealth tools and EMR usage are integral, with a 90-day probationary period and a retention bonus program in place.
Qualifications
- RN with at least 3 years of healthcare experience.
- Valid Ohio professional license required.
- Behavioral health experience strongly preferred.
- Experience with EMR and telehealth tools is preferred.
Responsabilités
- Provide patient-centered nursing care for caseload.
- Update individualized treatment plans as medications are used.
- Coordinate with team and communicate with patients and referrals.
- Perform medication reconciliation and ensure accurate documentation.
- Educate patients on medications and behavioral health conditions.
- Ensure proper medication storage and disposal per policy.
- Use EMR and video conferencing to support patient care.
