A home health care provider is seeking a full-time Assistant Clinical Manager (RN) in Yukon, Oklahoma. This role involves overseeing daily clinical operations, developing patient care plans, and mentoring nursing staff. The ideal candidate should have an active RN license in Oklahoma, at least 2 years of home health experience, and excellent organizational skills. Qualifications

• Maintain strong referral source relationships and promote positive patient outcomes.

What You'll Do as an Assistant Clinical Manager

In this leadership role, you'll support the Clinical Manager in overseeing daily clinical operations and serve as a key connection between referral sources, patients, caregivers, and staff. Your responsibilities include:

• Assisting with the development and revision of individualized patient care plans
• Responding promptly to staff and physician calls regarding orders or care concerns
• Ensuring accurate entry of orders, medication updates, and patient information
• Leading weekly case conferences and supporting decisions based on clinical findings
• Maintaining strong referral source relationships and promoting positive patient outcomes
• Entering referrals into Home Care Home Base accurately and promptly
• Assisting with scheduling and occasionally providing direct patient care
• Participating in interviewing, hiring, evaluations, and disciplinary actions
• Practicing infection control and reporting any suspected abuse per state and federal guidelines
• Demonstrating reliability, professionalism, and a commitment to high‑quality care

What We're Looking For
