Job summary

At Duly Health and Care, the Anticoagulation Clinic Nurse focuses on managing anticoagulation therapy, adjusting warfarin doses, and counseling patients. The role supports both in-office and telephone management to maintain safe therapeutic dosing.

The position requires RN licensure in Illinois, certification in anticoagulation management within 6 months, and BLS. 2 years in acute care or anticoagulation is preferred, with BSN as a plus.

Qualifications

• Graduation from an accredited Registered Nursing program with an Illinois RN license.
• Anticoagulation Therapy Management Certification within 6 months of hire.
• Current BLS certification.
• State of Illinois license certification provided by NBAC.
• BSN preferred.
• 2 years of experience in acute care or anticoagulation management.
• Strong knowledge of anticoagulation medications.
• Ability to anticipate and meet patient/family needs.

Responsibilities

• Assess, dose, and plan care for patients on anticoagulation therapy.
• Perform fingerstick INRs for warfarin patients in the office.
• Use protocols and dosing algorithms to adjust therapy as needed.
• Make clinical decisions to manage anticoagulation during illnesses or surgeries.
• Reassess and modify plans after reviewing results and history.
• Consult with the managing provider for direction per protocols.
• Educate patients and families at referral and ongoing education.
• Handle office tasks: documentation, records, and patient outreach.
• Process prescription refills per protocol.
• Outreach to overdue patients for testing.
• Comply with department policies and health and safety requirements.

Position Overview

The focus of this position is managing anticoagulation therapy. This involves adjusting medication dosage based on the INR lab results, developing peri-operative plans for medication interruption, responding to critical lab values and counseling patients on a full range of issues pertaining to anticoagulation. This includes indication for therapy, laboratory tests for monitoring, signs and symptoms of hemorrhagic or thromboembolic complications, dietary interactions, alcohol consumption, medication interactions and how underlying medical conditions affect anticoagulation therapy. The Anticoagulation Clinic Nurse will assist anticoagulated patients both in the office and via telephone management to assist maintaining a safe and therapeutic dosing regimen of warfarin.

Key Responsibilities

• Demonstrates a high level of autonomy and expert skill in the assessment, dosing and plan of care for those patients taking anticoagulation medications
• Performs fingerstick INRs for warfarin patients seen in the office
• For all INR results received, including from external sources, uses established protocols and dosing algorithms, experience, education and critical thinking skills to make dosing changes as necessary to assist in maintaining patient’s therapeutic regimen
• Formulates clinical decisions to manage anticoagulation during acute illnesses and surgeries
• Reassesses and modifies plans as needed after review of results and history with each result.
• Confers with managing provider for clinical direction as outlined by protocols
• Provides initial education to patients and their families upon referral to the ACC and continues to educate patients as needed, allowing them to be active participants in their care
• Manage office responsibilities which include documentation, records, and patient outreach
• Processes prescription refill requests per protocol
• Completes outreach to patients who are overdue for testing
• Complies with established department-specific policies, procedures, and objectives
• Complies with all health and safety regulations and requirements

Experience and Qualifications

• Graduation from an accredited Registered Nursing program with current and valid Illinois RN license
• Anticoagulation Therapy Management Certification Course within 6 months of hire
• Current BLS certification
• State of Illinois License Certification provided by the National Certification Board for Anticoagulation Providers
• Graduate of an accredited Registered Nursing program
• Bachelor of Science in Nursing preferred
• 2 years of experience in an acute care setting or specific anticoagulation management
• Strong knowledge of anticoagulation medications.
• Ability to anticipate, recognize, and meet the needs of patients and their families.

The compensation for this role includes a base pay range of 25.94- 38.91, with the actual pay determined by factors such as skills, experience, education, certifications, geographic location, and internal equity. Additional compensation may be available through shift differentials, bonuses, and other incentives. Base pay is only a portion of the total rewards package.
