Job summary

Qualifications

Responsibilities

• Identify patients for proactive interventions using screening criteria and referrals.
• Develop, maintain, and monitor patient care plans aligned with NCQA policies.
• Support patient self-management using coaching techniques and assess readiness for change.
• Educate care team and patients about case management processes and referrals; advocate for patient rights.
• Collaborate with physicians and health-team members to ensure timely care and resolve delays.
• Build partnerships with hospital, post-acute providers, and other care managers for seamless transitions.
