Job summary

Heart’n Soul Hospice is seeking a Registered Nurse for after-hours coverage in Memphis, TN. This role focuses on providing compassionate home visits, independent patient assessments, and coordination with physicians and care teams to ensure high-quality end-of-life care.

Ideal candidates will have an accredited nursing degree, an active RN license, and strong communication skills. The role includes comprehensive benefits and requires a valid driver's license for travel to patient homes.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Two years of RN experience preferred in a healthcare/clinical setting.
• Current Driver's License and reliable transportation.

Responsibilities

• Provide home visits for patient assessment, treatment, and support, including crisis management and pronouncement of death when needed.
• Develop and revise nursing care plans with the physician and IDG.
• Communicate effectively with patients, families, and internal teams; maintain proper phone etiquette.
• Prepare progress notes showing advancement toward goals.
• Inform physicians of changes in patient needs and outcomes; monitor patient/family response to care.
• Submit all documentation in a timely manner.
• Perform additional duties as required.
• Ensure compliance with local, state, and federal regulations and agency policies.

What You Must Have:

• Two years of experience as an RN preferred in a healthcare/clinical setting

What You Will Do:

• Provide home visits as needed for patient assessment, treatment, and support. Visits may include pronouncement of patient death, crisis management and admission
• Plans for comprehensive service to patients and their families; develops and/or revise nursing care plans, including goals in conjunction with the patient's physician and IDG
• Utilize excellent customer care skills with internal and external customers, including using proper phone etiquette
• Prepare clinical and progress notes that demonstrate progress toward established goals
• Inform physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
