Qualifications

Responsibilities

• Acts as a liaison among physicians, case managers, hospitals, and home health agencies to support inpatient admission.
• Initiates patient and family orientation, education, and tours.
• Coordinates with admissions and business offices to ensure admission criteria meet government agencies and third-party verification.
• Plans and participates in developing marketing goals and program outreach within the service area.
• Maintains an admission/denial log and makes recommendations for alternative placement when needed.
• Arranges transportation and insurance verifications as part of the admission process.
