Bishop Rehabilitation & Nursing Center in Syracuse, NY is seeking an RN Admissions Nurse to join our facility, covering the 11 AM–7 PM shift. The role focuses on completing admission assessments, initiating care plans, and coordinating with the interdisciplinary team to ensure a seamless transition for new residents.

The ideal candidate holds an active NYS RN license, brings strong assessment skills and the ability to work independently, and has experience in long-term care.

• Experience as Admissions Nurse in long-term care.

• Ability to make sound decisions and work independently.
• Strong leadership and supervisory skills.
• Comfortable using computers and MS Office.
• Long-term care experience preferred.
• Strong communication and people skills.

Responsabilités

• Complete admission assessments for new residents and hospital returns, including starting the initial plan of care.
• Support and guide the Nursing Department as directed by the Director of Nursing.
• Review PRI information and coordinate admission with the interdisciplinary team.
• Review resident records and complete audits to ensure facility standards and policies are followed.
• Review and update resident care plans when conditions change.
• Assist with discharge planning as needed.
• Complete required documentation and charting timely.
• Complete MDS assessments and related paperwork.
• Oversee and direct resident care when needed.
• Maintain resident privacy per HIPAA and confidentiality guidelines.

11AM-7PM

Duties & Responsibilities:

• Complete admission assessments for new residents and hospital returns, including starting the initial plan of care.
• Help support and guide the Nursing Department as directed by the Director of Nursing.
• Review PRI information and work with the interdisciplinary team to help coordinate the admission process.
• Review resident records and complete audits to make sure facility standards and policies are being followed.
• Review and update resident care plans when there are changes in the resident’s condition.
• Assist with discharge planning as needed.
• Complete all required documentation and charting in a timely manner.
• Complete MDS assessments, care plans, and other required paperwork as assigned.
• Help oversee and direct resident care when needed.
• Maintain resident privacy and follow all HIPAA and company confidentiality guidelines.

Requirements & Qualifications:

• Must have an active and unrestricted NYS Registered Nurse (RN) license.
• Previous experience working as an Admissions Nurse in a long-term care setting.
• Strong clinical assessment and critical-thinking skills.
• Able to make sound decisions and work independently.
• Strong leadership and supervisory skills.
• Comfortable using computers and Microsoft Office programs, including Word, Excel, and Outlook.
• Long-term care experience is preferred.
• Strong communication and people skills.
