Job summary

Circle of Life is seeking an Admission/Liaison Nurse to coordinate hospice admissions for patients and families in NE Oklahoma and Northwest Arkansas. You will partner with the Medical Director and the Interdisciplinary Team to develop comprehensive care plans and ensure timely, compassionate admissions.

This role requires daily travel, excellent clinical skills, and the ability to educate families and train caregivers.

Qualifications

• Current unrestricted AR RN license
• BLS certification
• Strong assessment skills with critical thinking and problem solving in healthcare
• Excellent documentation and communication skills
• Must travel daily with own vehicle, valid license and auto insurance

Responsibilities

• Coordinate and execute hospice admissions with Interdisciplinary Team and Medical Director
• Communicate hospice concepts to families and patients
• Develop care plans and communicate patient/family needs to the team
• Educate patients/caregivers and train on care tasks
• Schedule and coordinate disciplines to meet patient/family needs
• Perform head-to-toe assessments and consult with physician

Job description

This position will serve patients and families in NE Oklahoma and NWA. We have a variety of shift configurations including nights, days, and weekends.

Requirements

Current unrestricted AR RN license, BLS certification, strong assessment skills with critical thinking and high level of problem solving within a healthcare facility. Strong documentation and communication with the ability to stay calm and cope with human suffering/emergencies. Prefer hospice experience, home health, or acute care nursing experience with computer charting experience. Position requires use of personal vehicle, a valid driver's license and current auto insurance.

General Description

The Admission/Liaison Nurse is an integral part of the Interdisciplinary Team and works closely with the Medical Director for determining and facilitating hospice admissions. Primarily responsible for coordination and execution of the admission process into Hospice care. Nurse will need to be able to effectively communicate the Hospice concept to families and patient. Will develop the plan of care and effectively communicate the patient/family care needs to the team, must be able to train and educate patient/caregivers, and establish an appropriate frequency of disciplines to meet the patient/families' needs. Must be able to deal with human suffering while maintaining professional boundaries. Required to be flexible and change schedule at a moment's notice to meet patient needs. Required to travel daily and manage own schedule effectively. Must have excellent clinical skills and be able to identify problems, perform head to toe assessment, educate and consult with the physician, and execute orders to meet patient's needs.
