Position: RN Admissions - Clinical Assessment Advisor

Responsibilities include directly completing and overseeing development of treatment plans approved by physicians, performing ongoing patient evaluations, and coordinating a team of professionals and licensed clinicians to ensure optimal clinical outcomes. The role involves working with physicians and referral sources to gather clinical information, ensuring team care scheduling, disseminating updated clinical data, and maintaining focus on healthcare quality and affordability initiatives (HCQAIs). Performance will be monitored under supervision.

Essential Functions

1. Evaluate patient needs initially and ongoing within scope of practice.
2. Complete Start of Care, Resumption of Care, Recertification, and Discharge visits, with occasional routine visits based on patient needs.
3. Incorporate health maintenance, prevention, and safety education into care plans.
4. Manage care resources to meet outcomes.
5. Reassess and revise care plans as necessary.
6. Ensure appropriate care through assessments.
7. Facilitate communication among team members, physicians, and external parties to ensure appropriate care and updates.
8. Assign qualified caregivers to cases.
9. Participate in special projects and perform additional duties as assigned.

Required Skills and Experience

• Bachelor of Science in Nursing (preferred).
• Minimum 2 years clinical home care experience, with at least 1 year in a management role (preferred).
• Current CPR certification.
• Excellent communication and interpersonal skills.
• Knowledge of home health regulations, clinical policies, and PDGM structure.
• Fluent in English.
• Reliable attendance and transportation, valid driver’s license, auto insurance.

Compensation and Benefits

Annual salary ranges from $69,800 - $96,200, with potential bonuses based on performance. Benefits include medical, dental, vision, 401(k), paid time off, disability, life insurance, and more.
