They are dedicated to ensuring a seamless and efficient transition for patients coming in and out of acute care hospital settings. Specialized assessment skills help ensure that patients are placed in the appropriate care areas and discharged. We offer education and post-hospitalization care as necessary. Their main focus is advocating for patients, families, and caregivers while embodying the organization's vision, mission, and values.
