TriStar Greenview Regional Hospital in Bowling Green, KY is seeking an RN Admission-Discharge to deliver direct patient care within the scope of the Registered Nurse Practice Act. You will perform discharge planning, teach patients and families, and coordinate care with the care team to ensure safe transitions.

The role requires nursing school graduate, 2 years med-surg experience, TN RN licensure, BLS, ACLS and CPI training within 6 months.

Qualifications

• Graduate of an Accredited School of Nursing
• 2-years med-surg experience required, cardiac exp preferred
• Current TN RN licensure
• BLS
• ACLS required within 6 months
• CPI certification training required within 6 months

Responsibilities

• Obtains a nursing assessment history and facilitates appropriate admission actions
• Completes admission, risk, and discharge documentation and related post-hospital records
• Teaches patients/families about condition, treatment plan, medications and follow-up
• Completes discharge documentation and education for special patient populations
• Ensures copies of discharge paperwork and referrals are provided to patients and families
• Coordinates with House Supervisors/Nursing Administration to prioritize workload and ensure smooth transitions
• Demonstrates prioritization of high-priority activities

Job Summary And Qualifications

The Registered Nurse performs direct patient care within the scope of the Registered Nurse Practice Act. Utilizes therapeutic communication in daily practice. Maintains a safe therapeutic environment. Identifies nursing problems as they relate to the individual needs of the patient. Plans individualized patient care utilizing an interdisciplinary approach. Participates in performance improvement activities. Provides care and coordinates participation that fosters the goals of the hospital's mission and vision statement. Discharge Lounge RN is responsible for discharge teaching and discharge instructions at the time of discharge. Discharge Lounge RN is also responsible for effective communication and collaboration with ancillary departments.

What will you do in this role:

• Obtains a nursing assessment history, and ensures proper inpatient care unit admission. Takes appropriate action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect.
• Completes and facilitates the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.
• Teaches patients/families/caregivers about a patient’s medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures. Translates complex medical terminology to ensure complete knowledge.
• Completes extra discharge documentation, education, and discharge care for special patient populations, to ensure compliance with core measures for discharge.
• Ensures patients /families/caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc.
• Coordinates with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.
• Demonstrates a time-conscious awareness and ability to prioritize. Distinguishes between low and high-priority activities.

What qualifications you will need:

• Graduate of an Accredited School of Nursing
• 2-years med-surg experience required, cardiac exp preferred
• Current TN RN licensure
• BLS
• ACLS required within 6 months
• CPI certification training required within 6 months
