Job summary

SAINT BERNARDS MEDICAL CENTER in Arkansas seeks a Registered Nurse to expand Hospital at Home and Transitional Care. The RN will provide hospital-level nursing care in patients' homes, including IV medications, wound care, oxygen management, and coordination of mobile diagnostics and supportive services.

The role follows patients for 30 days after discharge, coordinating care with physicians, nurse practitioners, and the broader team via telemedicine and other tech.

Qualifications

• Must be licensed as a Registered Nurse in Arkansas.
• Minimum of one year in an acute care setting.
• Ability to care for geriatric patients with strong clinical judgment.

Responsibilities

• Provide hospital-level nursing care in the home setting.
• Coordinate mobile diagnostics, equipment, and services.
• Administer IV meds and complex wound care as prescribed.
• Manage oxygen therapy and other skilled nursing interventions.
• Follow patients for 30 days post-discharge and coordinate care.
