Qualifications

• Must be currently licensed as a Registered Nurse in the state of Arkansas.
• Minimum one year of experience in an acute care setting.
• Demonstrated knowledge and skills to provide care to geriatric patients and communicate with pediatric and adult populations.

Responsibilities

• Provide hospital-level and transitional care in the home setting.
• Coordinate mobile diagnostics, medical equipment, and supportive services such as physical therapy and social work.
• Follow assigned patients for 30 days post-discharge, coordinating care and preventing readmissions.
• Communicate with physicians, nurse practitioners, and the interprofessional team via telemedicine and other technologies.
