Job summary

State of New York - People With Developmental Disabilities, Office for Taconic in Dutchess County seeks a Registered Nurse 2 – Quality Improvement (Community). This RN2 will support the district’s quality improvement efforts as a nursing subject matter expert, ensuring standards across the district.

The role requires clinical experience, community-based work, and collaboration with state operations and the QI department.

Qualifications

• NY State RN license and current registration required.
• Two years post-licensure clinical nursing experience.
• At least one year direct service experience with individuals with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.

Responsibilities

• Complete medical charts and site reviews at residential day habilitation sites.
• Complete E.H.R. review of individual medical charts.
• Provide training, technical assistance, site visits and audits for nursing and residential staff; monitor service delivery documentation and outcomes.
• Complete DQI survey readiness reviews prior to DQI visits.
• Review exit conferences and develop nursing-related POCAs with state operations teams.
• Participate in Nursing Standards of Practice and Health Service work groups to assist in SOP development.
• Complete reviews of incidents/investigations (medical, falls, deaths) as assigned.
• Serve on Incident Review, Medication Certification Task Force, and Mortality Review committees.
• Other duties as assigned.
