Job summary

Optalis Healthcare (Health & Rehabilitation Centers) in Southfield, MI seeks a Registered Nurse capable of coordinating resident care and supervising clinical staff to maintain high-quality standards. You will work with the Administrator and Director of Nursing to implement care plans, administer medications, respond to calls, and ensure compliance with CMS/state regulations.

The role requires strong communication, organization, and a resident-centered approach to help residents achieve optimum

Qualifications

• Bachelor's or Associate's degree in Nursing from an approved School of Nursing
• Possess a Registered Nurse License from the State that is in good standing
• 2 years experience in long term care and in the management of resident care preferred

Responsibilities

• Coordinate resident care in conjunction with Administrator, Director of Nursing and other disciplines within Center to ensure quality care. Ensure care plans are implemented and updated based on resident needs.
• Engage and partner with others to develop care plans and provide continuity of care to facilitate best outcomes for residents and clients.
• Administer scheduled medication to residents in a timely manner, ensuring proper and correct dosage is given. Support survey readiness and quality assurance activities.
• Answer call lights and communicate needs to appropriate person when unable to independently meet the request. Prioritize call light requests and ensure timely resolution.
• Strong clinical assessment and supervisory skills.
• Knowledge of CMS/state regulations and SNF standards.
• Effective communication and documentation abilities.
• Organizational and time management skills.

The purpose of this job is to ensure residents receive appropriate personal care and are enabled to attain their optimum level of functioning. To ensure that the center remains in compliance with organization, local, state, and federal regulations.

Physical & Work Requirements
