Optalis Healthcare is seeking a licensed nurse (LPN/RN) to coordinate resident care and ensure adherence to care plans and regulatory standards in a long-term care setting. The role emphasizes communication, documentation, and timely response to resident needs.

The candidate should have a valid state license, a high school diploma/GED, and CPR certification, with a preference for 2 years in long-term care management of resident care. This is a full-time on-site position.

Qualifications

  • Ability to lift 80--300 lbs with assistance/equipment.

Responsibilities

  • Coordinates resident care with Administrator, Director of Nursing and other disciplines to ensure quality care and updates to care plans based on resident needs.
  • Exercises effective decision making to maximize best clinical outcomes for residents.
  • Answers call lights and communicates needs when unable to meet requests; prioritizes calls and ensures timely response.
  • Completes daily rounds and provides shift reports that are accurate and relevant to residents’ care.
  • Supports survey readiness and quality assurance activities.
  • Maintains dissemination of resident information in a timely and appropriate manner.
  • Demonstrates strong customer service, reliability, accountability, and effective communication/documentation skills.
  • Exhibits organizational and time management abilities.

Job description

The purpose of this job is to ensure residents receive appropriate personal care and are enabled to attain their optimum level of functioning. To ensure that the center remains in compliance with organization, local, state and federal regulations.

Key Responsibilities:
  • Coordinates resident care in conjunction with Administrator, Director of Nursing and other disciplines within Center to ensure quality care. * Ensure care plans are implemented and updated based on resident needs.
  • Exercises effective decision making to maximize best clinical outcomes for residents.
  • Answer call lights and communicate needs to appropriate person when unable to independently meet the request. Demonstrates ability to prioritize call light request and to make certain all are taken care in a timely manner* Support survey readiness and quality assurance activities.
  • Completes daily rounds with incoming shift. Provides shift report to oncoming personnel, which is accurate, concise and pertinent to the resident's care. Coordinates the dissemination of resident information in an appropriate and timely manner
