Job summary

Palms-Home-Care is hiring registered nurses for full-time and PRN home health positions in Orlando, FL. You will perform patient assessments, develop care plans, review meds and vitals, and coordinate with physicians and therapists. EHR experience with Kinnser is preferred, and you’ll educate patients to promote independence.

Travel to patient homes is required, and exceptional communication with the care team is essential to ensure high-quality outcomes.

Qualifications

• Active Florida RN license in good standing.
• Minimum 1 year home health nursing experience.

Responsibilities

Reports To: Clinical Manager

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans in collaboration with the care team.
• Complete evaluation tasks, including reviewing medication and vital signs.
• Coordinate care by making appropriate referrals or documenting clinical rationale when referrals are not indicated.
• Communicate effectively with the clinical manager and interdisciplinary team regarding patient status and care updates.
• Educate patients and caregivers to promote independence and improve health outcomes, including training in self-care techniques.
• Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
• Supervise and support LPNs to ensure high standards of care.
• Perform additional duties as needed to support patients and team operations.

Qualifications

• Current Registered Nurse (RN) license in the state of Florida
• Must have at least 1 year experience as a home health nurse
• Excellent knowledge of nursing care methods and procedures
• In-depth knowledge of health and safety guidelines and procedures
• A team player with excellent communication and interpersonal skills
• Outstanding organizational and multi-tasking skills
• Patient with excellent problem-solving skills
• Familiar with EHR such as Kinnser.
• Ability to work independently, manage a daily schedule, and travel between patient homes
