Job summary

Haggai Healthcare in Gilbert, AZ is seeking a Home Health Registered Nurse (RN) to deliver compassionate in-home care and support for patients in the surrounding area.

As an RN you will collaborate with the care team, assess progress, design care plans, and manage discharges, all while setting your own patient load and schedule to achieve a healthier work-life balance. Licensure required; home health experience preferred but not mandatory.

Responsibilities of the Home Health RN - Registered Nurse

• Effectively collaborate with care team
• Patient assessment and evaluation of patients progress towards goals

• Plan and facilitate appropriate discharge

• Provide care which promotes quality, continuity, and patient safety
• Provide skilled nursing care to patients in the community
