Job summary

Resources for Human Development in Delaware is seeking a Registered Nurse to join the Assertive Community Treatment (ACT) Team. You will coordinate healthcare services, manage medications, support wellness, and advocate for clients throughout their recovery journey.

This full-time role involves delivering community-based nursing, collaborating with a multidisciplinary team, and traveling within the service area to provide person-centered behavioral healthcare.

Qualifications

  • Current and unrestricted Registered Nurse (RN) license in the State of Delaware.
  • Graduate of an accredited School of Nursing; CPR Certification.
  • Successful completion of required background investigations and TST.
  • Valid driver's license with reliable transportation.

Responsibilities

  • Conduct comprehensive psychiatric, clinical, and physical health assessments.
  • Monitor client health status, treatment progress, and medications.
  • Administer medications and injections; document nursing interventions.
  • Coordinate services with physicians, psychiatrists, and other providers.
  • Support community-based nursing services and health education.
  • Participate in crisis intervention, care planning, and referrals.
  • Ensure HIPAA compliance and accurate clinical documentation.
  • Provide leadership and training to staff on health monitoring.

About the ACT Team

The Assertive Community Treatment (ACT) Team provides comprehensive, community-based behavioral health services to individuals with serious mental illness and co-occurring disorders. Our team works collaboratively to ensure clients receive the support, treatment, and resources they need to successfully live, work, and thrive in their communities.

As an ACT Team RN

You will play a vital role in coordinating healthcare services, managing medications, supporting wellness, and advocating for individuals throughout their recovery journey.

What You’ll Do

As a Registered Nurse on the ACT Team, you will:

Clinical Nursing Care
  • Conduct comprehensive psychiatric, clinical, and physical health assessments
  • Monitor client health status and treatment progress
  • Administer medications, injections, and prescribed treatments
  • Monitor medication effectiveness, side effects, and treatment outcomes
  • Maintain medication records and ensure regulatory compliance
  • Complete nursing interventions, screenings, and health monitoring activities
Community-Based Support
  • Provide nursing services in homes and community settings
  • Support medication self-management and independent living skills
  • Promote wellness, disease prevention, and health education
  • Deliver individualized interventions that support recovery and self-sufficiency
Care Coordination
  • Coordinate services with physicians, psychiatrists, specialists, pharmacies, and hospitals
  • Assist clients with accessing healthcare and community resources
  • Facilitate referrals and follow-up care
  • Build strong partnerships with healthcare providers and support systems
Crisis Intervention & Treatment Planning
  • Respond to psychiatric, behavioral health, and medical crises
  • Participate in interdisciplinary treatment planning meetings
  • Collaborate with psychiatrists, therapists, case managers, and peer specialists
  • Provide clinical recommendations and nursing expertise to the treatment team
Documentation & Compliance
  • Complete accurate and timely clinical documentation
  • Maintain compliance with nursing standards, HIPAA requirements, and agency policies
  • Support infection control and quality improvement activities
Leadership & Advocacy
  • Provide clinical consultation to team members
  • Support and train staff on medication administration and health monitoring
  • Advocate for client rights, access to care, and community integration

QualificationsRequired Qualifications
  • Current and unrestricted Registered Nurse (RN) license in the State of Delaware
  • Graduate of an accredited School of Nursing
  • CPR Certification
  • Successful completion of a pre-employment Tuberculin Skin Test (TST)
  • Ability to obtain and maintain Delaware Department of Correction security clearance
  • Successful completion of all required background investigations
  • Valid driver's license, current vehicle registration, and reliable transportation
Preferred Qualifications
  • Bachelor of Science in Nursing (BSN)
  • Minimum of one year of nursing experience
  • Experience in behavioral health, community mental health, ACT programs, substance use treatment, or integrated healthcare settings
  • Experience providing community-based services and care coordination
Schedule
  • Full-Time Position
  • Community-Based Role with Local Travel
  • Flexible schedule based on client needs
  • Participation in on-call rotation as required
Working Conditions
