Job summary

Resources for Human Development (RHD) is seeking a compassionate Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team in Claymont, DE. This is more than a nursing position—it's an opportunity to help individuals living with mental health, substance use, and developmental disabilities achieve recovery in their communities.

You will conduct comprehensive nursing assessments, coordinate care, and support wellness plans.

Qualifications

  • Must have an Associate's or Bachelor's Degree in Nursing.
  • Current Delaware RN license in good standing or a Compact License.
  • Current CPR and First Aid Certification.
  • Valid driver's license with at least three years of driving experience.
  • Strong communication, documentation, and organizational skills.
  • Ability to work with individuals experiencing mental health, substance use, and co-occurring disorders.
  • Strong attention to detail and ability to manage multiple priorities.

Responsibilities

  • Conduct comprehensive nursing and medical assessments.
  • Coordinate and facilitate healthcare services.
  • Develop, monitor, and support individualized treatment and wellness plans.
  • Monitor medication adherence, health status, and wellness outcomes.
  • Collaborate with participants, families, healthcare providers, and community partners.
  • Maintain accurate and timely clinical documentation.
  • Serve as a liaison between participants and external healthcare providers.
  • Participate in quality improvement and program evaluation activities.
  • Provide after-hours on-call support as required.
  • Ensure compliance with all professional nursing standards and regulations.
  • Promote recovery-oriented, person-centered care.

About the ACT Team

The Assertive Community Treatment (ACT) Team provides comprehensive, community-based behavioral health services through an interdisciplinary team approach. Team members include nurses, psychiatrists, counselors, substance use specialists, vocational specialists, peer support specialists, and program support staff.

Together, we help individuals:

  • Achieve recovery and wellness goals
  • Maintain physical and behavioral health
  • Access housing, employment, benefits, and community resources
  • Develop independent living and social skills
  • Improve overall quality of life and community integration

This team provides intensive support designed to help individuals achieve their highest level of independence while remaining active members of their communities.

What You'll Do

As a Registered Nurse (RN) – ACT Team, you will:

  • Experience using Electronic Health Records (EHR)
  • Knowledge of substance use treatment and recovery-oriented care
  • Proficiency with Microsoft Office and related business applications
Schedule
  • Full-Time
  • Community-based position with local travel
  • Participation in an on-call rotation required
  • Occasional evening or after-hours support based on program needs
