Job summary

AndHealth LLC is seeking an Advanced Practice Provider (APP) with rheumatology experience to join our onsite care team in Indianapolis, IN. The APP will help with diagnosis, treatments, and consultations, delivering a whole-person approach within a collaborative care model.

You will collaborate with internal and external providers, support a clinical protocol, and participate in ongoing innovation of AndHealth’s care model while ensuring patient access to comprehensive care.

Qualifications

• Active license to practice without restrictions in the state of placement.
• Master’s degree from an accredited program required.
• Certified Nurse Practitioner (NP) or Physician Assistant (PA-C) certification required.

Responsibilities

• Create personalized treatment plans for patients and implement their care plans.
• Utilize and support a clinical treatment protocol updated by the AndHealth team.
• Review Health Coach cases and provide feedback on treatment plans.
• Assess medication readiness and manage medications; order and interpret labs as needed.
• Coordinate care with coaches and other providers during clinical rounds and as escalation support.
