Job summary

AndHealth LLC in Indianapolis, IN, seeks an Advanced Practice Provider (APP) with medical dermatology experience to join our onsite care team. You will create personalized treatment plans, implement care, and collaborate with physicians, nurses, and coaches to deliver whole-person care.

The role emphasizes telehealth-enabled care and data-driven outcomes. Requires active PA/NP licensure, Master’s degree, and dermatology specialization preferred.

Qualifications

• Active license to practice in the state where the position is based.
• Certified NP or PA-C.
• Master’s degree from an accredited program.
• Practical specialization in Medical Dermatology preferred.

Responsibilities

• Create personalized treatment plans and work directly with patients in implementing their care plan.
• Utilize and support a detailed clinical treatment protocol.
• Review Health Coach managed cases and provide feedback on treatment plans.
• Assess medication readiness/eligibility and manage medications; order and interpret labs as needed.
• Review and oversee patient progress; coordinate care with coaches and other team members.
• Collaborate with providers internal and external to ensure access to care.
• Participate in innovating and measuring the care model.
