Mount Carmel Health System in Ohio seeks a Patient Care Pharmacist to provide timely, cost-effective, patient-centric pharmaceutical care within the scope of the Ohio Pharmacist Practice Act. The role emphasizes accurate dispensing, regulatory compliance, and collaboration with a multidisciplinary team.

The candidate should hold a Bachelor or Doctor of Pharmacy, be licensed in Ohio, and preferably have PGY1 residency or several years in institutional pharmacy.

Qualifications

• Pharmacist license in Ohio (Registered Pharmacist) required
• PGY1 residency or 3 years in institutional pharmacy in an advanced clinical setting preferred
• Strong communication and organizational skills
• Ability to plan, organize and manage pharmaceutical care, including delegating tasks
• Performs work independently with minimal supervision, remains effective in stressful environments

Responsibilities

• Coordinate preparation, distribution, and dispensing of medications in compliance with laws and policies; direct pharmacy staff
• Maintain compliance records and regulatory readiness; track transactions and accountability for drugs
• Assist in inventory management; manage non-formulary requests and stock quality
• Interpret, evaluate and verify all medication orders
• Collaborate with multidisciplinary team to optimize drug therapy and monitor for interactions and adverse effects

About the job:

The Patient Care Pharmacist, within the scope of the Ohio Pharmacist Practice Act, will provide timely, accurate, cost effective, and patient-centric pharmaceutical care.

What we are looking for:

• Education: Bachelor or Doctor of Pharmacy required
• Licensure / Certification: Current license to practice as a Registered Pharmacist in the State of Ohio. Preferred - PGY1 residency or three years of institutional pharmacy experience in an advanced clinical setting
• Effective Communication Skills
• Demonstrates the ability to plan, organize and manage pharmaceutical care, including delegating tasks to and providing direction to other members of the pharmacy team
• Performs work independently with minimum supervision, and is effective in a stressful environment

What you will do:

• Coordinates the preparation, distribution, and dispensing of medication as appropriate in compliance with state and federal regulations and laws, accreditation standards, and departmental policies and procedures; directs and assists pharmacy support staff.
• Maintains personal compliance and assists in maintenance of departmental compliance with continuous regulatory readiness. Also maintains records related to the transactions of the pharmacy, to achieve adequate control and accountability for all drugs, including controlled substances.
• Assists in the management of pharmacy inventory by providing information to pharmacy purchasing, removing expired contaminated or deteriorated stock when detected, helping maintain perpetual inventories, and processing non-formulary requests.
• Interprets, evaluates and verifies all medication orders.
• Collaborates with multidisciplinary team in order to assure optimal and cost effective drug therapy. This includes adjusting drug dosing and schedules, recommending the most efficacious therapy, practicing stewardship in antibiotics, performing drug consults and monitoring, and detecting possible adverse drug reactions, food/drug interactions, drug/drug interactions, and allergic cross reactivity.
