We are seeking a detail-oriented, compassionate, and driven Clinical Data & Transitions Specialist to join our growing team. This hybrid role combines clinical data management and administrative responsibilities with patient outreach and care coordination. Position Overview
The Clinical Data & Transitions Specialist plays an important role in supporting patient enrollment, clinical data accuracy, and seamless transitions into TeleMate Health’s remote patient monitoring (RPM) services.
This position requires a balance of administrative precision, clinical understanding, and compassionate patient communication. Responsibilities include reviewing patient records, entering and verifying clinical and demographic information, identifying relevant diagnoses, confirming insurance information, and conducting outbound calls to educate patients and facilitate enrollment.
The successful candidate is organized, diligent, and self-motivated, with the ability to work independently in a remote environment while communicating effectively and collaborating with multiple teams. Flexibility is essential, as responsibilities may shift based on operational needs.
Key Responsibilities
Clinical Data & Administrative Support
- Accurately enter, review, and maintain patient demographic, clinical, and administrative information across multiple electronic medical record (EMR) systems and platforms.
- Navigate patient charts, medical histories, and clinical documentation to locate relevant information and support patient eligibility review.
- Review documented diagnoses and ICD-10 codes to identify potential qualifying conditions for remote patient monitoring services.
- Identify missing information, discrepancies, or inconsistencies across patient records and follow established procedures to resolve or elevate them.
- Maintain accurate, organized, and timely documentation while meeting established productivity and quality expectations.
- Assist with patient intake processes, ensuring required information and documentation are complete.
- Verify patient insurance eligibility and relevant coverage information, following established workflows.
- Communicate with internal teams to clarify documentation, resolve data discrepancies, and support accurate patient records.
- Maintain patient confidentiality and comply with applicable privacy requirements and company policies.
Patient Transitions & Enrollment
- Make outbound calls to new and eligible patients to introduce TeleMate Health’s services and facilitate enrollment.
- Clearly explain the purpose, benefits, expectations, and functionality of remote patient monitoring services and equipment.
- Guide patients through the intake and enrollment process, obtaining and confirming necessary information.
- Help patients understand how and when to use their monitoring equipment and what to expect from the service.
- Build rapport with patients, address questions and concerns, and encourage participation through patient-centered communication.
- Provide a high level of customer service while demonstrating patience, empathy, and professionalism.
- Communicate relevant patient information and enrollment updates to the appropriate internal teams.
- Support a smooth transition into services by ensuring patient expectations are clear and necessary next steps are communicated.
Cross-Functional Team Support
- Collaborate with clinical, transitions, and other operational teams to support patient care and company objectives.
- Adapt to changing priorities and assist with responsibilities across teams as operational needs arise.
- Maintain clear, timely communication with colleagues and leadership, particularly when addressing barriers, delays, or patient concerns.
- Participate in training, team meetings, and process improvements to strengthen workflows and support a positive team environment.
- Demonstrate initiative, accountability, and a willingness to take ownership of tasks and contribute beyond day-to-day responsibilities when needed.
Qualifications & Skills
Required Qualifications
- Experience in a healthcare, clinical administrative, medical office, patient support, or similar role.
- Strong computer skills, including accurate data entry and the ability to navigate multiple software platforms.
- Excellent attention to detail and a commitment to maintaining accurate patient records.
- Strong verbal and written communication skills.
- Professional, compassionate, and patient-centered approach to customer service.
- Ability to manage multiple priorities and meet deadlines in a fast-paced environment.

- Reliable high-speed internet and a private, quiet home workspace where patient conversations can be conducted without interruption and protected health information remains confidential.
Preferred Qualifications
- Medical Assistant (MA) certification or experience in a similar clinical support role.
- Experience with electronic medical records (EMRs), electronic health records (EHRs), or other clinical documentation systems.
- Familiarity with ICD-10 codes, medical terminology, clinical diagnoses, and patient eligibility criteria.
- Experience with insurance eligibility verification, benefits review, or insurance-related administrative processes.
- Experience with clinical data entry, patient demographics, chart review, or medical record management.
- Experience working with geriatric patients or individuals who may need additional support navigating healthcare services.
- Experience in patient intake, care coordination, healthcare customer service, or transitions of care.
- Inside sales, patient enrollment, or other experience communicating service benefits and addressing objections.
- Contact center or high-volume outbound calling experience.
- Proficiency with Microsoft Office applications and other common workplace technology.
