Job summary

Medilodge of West Bloomfield is seeking a Certified Nursing Assistant to support residents in rehabilitation and restorative care, helping them achieve maximum independence.

You will work under the supervision of a licensed nurse, perform ROM, bed mobility, transfers and daily care tasks, and participate in care planning with the team to optimize resident well-being.

Qualifications

• High school diploma or GED (required).
• Graduate of a state-approved nursing assistant training program with competency exam.
• Six months’ experience in a long-term care setting preferred.
• Strong communication and teamwork skills.

Responsibilities

• Under supervision of a licensed nurse, initiate rehabilitation/restorative nursing interventions.
• Assist residents with ROM, bed mobility, transfers, walking, dressing, eating and grooming.
• Provide patient-centered care to promote independence and safety.
• Document objectives and resident responses in care plans and records.
• Collaborate with the care team to support residents’ functional goals.

Requirements

Summary:

Provides rehabilitation/restorative nursing interventions that promote the resident’s ability to adapt

and adjust to living as independently and safely as is possible. This concept actively focuses on achieving and

maintaining optimal physical, mental, and psychosocial functioning. Restorative nursing interventions, except for

use of Coninuous Passive Motion (CMP), do not require a physician’s order.

Qualifications:

Education:

• High school diploma or general education degree (GED).

Licenses/Certification:

• Graduate of a state approved nursing assistant training program and passed a state approved competency examination (written and skills) within 120 days of employment; name entered into state nurse aide registry.

Experience:

• Six months experience in a long-term care environment preferred.

Essential Functions:

• Under the supervision of the licensed nurse, initiates rehabilitation/restorative nursing interventions when:

A resident is discharged from formalized physical, occupational, or speech rehabilitation

therapy, or

A resident is admitted to the center with restorative needs, but is not a candidate for formalized

rehabilitation therapy, or

A restorative need arises during the course of a custodial stay

• Performs rehabilitation/restorative nursing interventions that assist or promote the resident’s ability to attain his or her maximum functional potential:

• Documents measurable objectives and interventions in the care plan and in the clinical record.
• Promotes resident’s ability to adapt to and adjust to living as independently as possible.
• Instructs the resident in self-care activities according to the resident’s abilities.
• Instructs the resident in the use of assistive devices for transferring, walking and mobility, dressing and grooming, eating and swallowing
• Provides range of motion, proper turning and positioning in bed and chair.
• Instructs the resident on the care and use of prosthetic and orthotic devices.
• Incorporates training and skill practice activities which include repetition, physical or verbal cueing and task segmentation.
• Reports observations and other pertinent nursing rehabilitation/restorative care information to nursing supervisor.
• Documents objective information related to the provision and resident response to nursing rehabilitation/restorative care in accordance with existing policies, procedures and guidelines.
• Participates in the ongoing review and development of the nursing rehabilitation/restorative care and interventions.
• Participates in the identification of staff and resident educational needs related to nursing rehabilitation/restorative care.
