Job summary

Alden Estates of Jefferson is seeking a dedicated Restorative Nurse to lead and supervise our restorative nursing program. You will promote a restorative philosophy across the facility and help participants achieve their highest level of function.

The ideal candidate will plan, implement, and evaluate restorative care services in accordance with current guidelines, oversee documentation in EMR, and supervise ADL-related activities, with a focus on reducing restraints and preventing falls.

Qualifications

• Current, unencumbered RN license in Wisconsin required.
• Restorative certification preferred.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct restorative care services and activities per applicable rules and guidelines.
• Set up documentation flow sheets and EMR for daily restorative nursing documentation.
• Oversee restorative nursing measures reflected on the care plan.
• Perform or oversee functional assessments on admission and quarterly; advise nurses on restorative care plans.
• Oversee ADL performance and CNAs documentation on ADL; manage incontinence programs.
• Support or lead physical restraint reduction and fall management programs.

Restorative Nurse

Alden Estates of Jefferson, a renowned post-acute care facility, is seeking a dedicated and compassionate Restorative Nurse to join our team! As a Restorative Nurse, you will play a vital role in developing, implementing, and supervising our restorative nursing program, promoting a restorative nursing philosophy among all facility departments, and ensuring participants achieve their highest level of function.

About the Role:

• Planning, developing, organizing, implementing, evaluating, and directing restorative care services, programs, and activities in accordance with current rules, regulations, and guidelines


• Setting up documentation flow sheets and electronic medical record systems for daily documentation of restorative nursing measures


• Overseeing all restorative nursing measures are reflected on the care plan


• Performing or overseeing functional assessments on admission and quarterly as indicated, consulting with nurses in making recommendations for restorative plan of care, and working with nurses in developing restorative plan of care


• Overseeing ADL performance on units, overseeing C.N.A.'s documentation on ADL's, and overseeing incontinent management programs


• Actively participating or overseeing physical restraint reduction programs and fall management programs

Requirements:

• Must possess a current, unencumbered license to practice as a Registered Nurse in the state of Wisconsin
