Cedar Ridge Health & Rehab Center in Lebanon, IL invites a licensed practical nurse (LPN) or registered nurse (RN) to join as Care Plan/Restorative Nurse. You will develop restorative nursing programs, collaborate with the interdisciplinary team, and monitor progress to improve resident independence.

Qualifications

• Active Illinois LPN or RN license is required.
• Experience in long-term care or restorative nursing preferred.
• Knowledge of MDS, care planning and restorative practices.

Responsibilities

• Develop and implement individualized restorative nursing programs and resident care plans in collaboration with the interdisciplinary team
• Monitor resident progress and update care plans as needed to meet changing resident needs
• Collaborate with therapy staff to ensure continuity and consistency of care
• Provide hands‑on restorative nursing care focused on mobility, ADLs, and rehabilitative support
• Promote resident independence, dignity, and functional improvement
• Ensure documentation is timely, accurate, and compliant with regulatory standards
• Educate residents and families on restorative goals and care techniques
• Support nursing staff in delivering resident-centered care that prioritizes recovery and well‑being
• Perform additional duties as assigned in a patient‑centered care environment
