Cedar Ridge Health & Rehab Center in Lebanon, IL is seeking a Care Plan/Restorative Nurse (LPN or RN) to develop and manage individualized restorative nursing programs for residents. You will work with an interdisciplinary team to promote independence, monitor progress, and ensure documentation meets regulatory standards.

Qualifications

• Current Illinois LPN or RN license.
• Experience in long-term care or restorative nursing preferred.
• Knowledge of MDS, care planning, and restorative nursing practices.
• Strong communication, organization, and teamwork skills.
• Passionate about helping residents achieve their highest level of function and independence.

Responsibilities

• Develop and implement individualized restorative nursing programs and resident care plans in collaboration with the interdisciplinary team.
• Monitor resident progress and update care plans as needed to meet changing resident needs.
• Collaborate with therapy staff to ensure continuity and consistency of care.
• Provide hands-on restorative nursing care focused on mobility, ADLs, and rehabilitative support.
• Promote resident independence, dignity, and functional improvement.
• Ensure documentation is timely, accurate, and compliant with regulatory standards.
• Educate residents and families on restorative goals and care techniques.
• Support nursing staff in delivering resident-centered care that prioritizes recovery and well-being.
• Perform additional duties as assigned in a patient-centered care environment.
