Job summary

Bel-Air & San Gabriel Rehabilitation is seeking a Restorative Aide to support restorative and rehabilitative care for ProCare I-SNP members as appropriate. The role focuses on personal care, mobility, and daily care tasks within an SNF setting.

Responsibilities include restorative nursing tasks, promoting patient independence, and contributing to care plans. CNA certification, English proficiency, and willingness to travel to additional SNF locations are required.

Qualifications

• State certification as a Nursing Assistant is mandatory.
• Ability to read, write, and speak the English language.
• Must be available to work six days per week
• Ability to travel to additional SNF locations within geographical area

Responsibilities

• Provide personal care to patients in a manner conducive to their safety and comfort.
• Performs restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding, etc., within the scope of individuals' training.
• Promotes and supports the greatest possible degree of independence for patients.
• Advises Director of Rehab (DOR) of Patient's status daily.
• Contributes knowledge of Patient's conditions/needs to patient care plans.
• Assists in the implementation of care plans as directed within the parameters of established policy and procedure.
• Observes and reports on Patients' conditions and documents observations as required.
