Job summary

Pivot Rehabilitation Services is seeking a Restorative Aide to support restorative and rehabilitative care for ProCare I-SNP members. The role involves providing personal care, assisting with restorative tasks, and promoting patient independence within policy guidelines.

Daily duties include reporting patient status to the Director of Rehab, contributing to care plans, and documenting observations. Travel to SNF locations within the geographical area is required.

Qualifications

• State certification as a Nursing Assistant is mandatory.
• Must be available to work six days per week.
• Must be able to travel to SNF locations within geographical area.
• Must read, write, and speak the English language.

Responsibilities

• Provide personal care to patients in a manner that ensures safety and comfort.
• Assist in restorative nursing tasks such as ambulation, range of motion, positioning, and self-feeding.
• Promote and support the greatest possible degree of independence for patients.
• Advise Director of Rehab (DOR) of patient status daily.
• Contribute knowledge of patient conditions to care plans and assist in their implementation.
• Observe and report on patients' conditions and document observations as required.
