Crown Point Health Campus is seeking a Restorative Aide to support restorative nursing and patient functioning. You will coordinate care under the Nursing department and Rehab teams, ensuring goals are met for each resident.

Ideal candidates hold Indiana CNA certification and are committed to high-quality patient care within a team-oriented environment. Open interviews available during posted hours.

Qualifications

• Must be a registered CNA in the State of Indiana.

Responsibilities

• Oversee and participate in restorative programs for patients.
• Communicate with Nursing Director and Rehab staff on goals and approaches.
• Assist transitions from skilled rehab to restorative programs.
• Attend rehab rounds and report patient status.
• Maintain documentation of restorative progress for all patients.
