Job summary

Melwood Community Services Inc is seeking a qualified candidate to coordinate person-centered respite services and provide comprehensive case management across State-Funded Services. You will ensure plans align with participant goals and comply with DDA regulations.

You will maintain accurate documentation in the ETO system, coordinate with families and community partners, and foster collaborative relationships with agencies throughout Maryland and Virginia.

Qualifications

• Bachelor's degree in a related field is required.
• Minimum of one year of experience in case management or human services.
• Strong organizational and communication skills; ability to protect confidentiality.
• Valid driver's license and ability to travel throughout Maryland and Virginia as needed.

Responsibilities

• Coordinate and facilitate person-centered respite services with aligned participant goals.
• Provide comprehensive case management for Respite and State-Funded Services.
• Maintain documentation in the Efforts to Outcomes (ETO) system and ensure quality records.
• Build collaborative relationships with state/local agencies, schools, and community organizations.
• Represent the organization at community events, resource fairs, and stakeholder meetings.
• Conduct participant meetings and communications using a person-centered approach.
