Using lived experience, you will guide and connect people served to community resources during intake, planning, and treatment, while supporting on-site and community-based participants.

• Provide people served with support, information, and assistance in connecting with and obtaining community-based resources and services throughout the program intake, Action Planning process and the course of treatment.
• Support persons served on site and in community settings.
