The Idaho Department of Health and Welfare is hiring a Resources and Services Navigator for the Pocatello office. The role partners with community resources to help families achieve stability, self-sufficiency, and safety, enabling children to be cared for at home or with relatives.

The position supports interview-based assessments, information/referrals, and timely service planning, with a strong emphasis on collaboration with partners and proper documentation.

Qualifications

• Bachelor’s degree or higher in a human services field OR at least three years of work experience in human services.
• Experience in case management including developing and monitoring case plans.

Responsibilities

• Identify individual and family needs and connect to community resources.
• Provide information, referrals, and diversions to meet identified needs.
• Identify and schedule assessments and gather collateral information for processing.
• Provide interim case management and referrals to reduce risk and meet goals.

The Service Integration – Navigation program has an opportunity for a Resources and Services Navigator. The ideal candidate will work closely with our community partners and create plans with families that promote stability, self-sufficiency, health, and safety so that children may be cared for in their own homes or in the homes of relatives. The position is located in ourPocatellooffice.

• Identify individual and family strengths and service needs through skillful interviewing and discussion with participants and review of enrollment forms and requests for service.
• Provide appropriate information, referral, and diversion to community resources to meet identified needs.
• Identify and schedule additional assessments, gather collateral information as needed for processing applications and for assessments.
• Provide interim case management to facilitate client access to support services; take action to reduce risk to client by making appropriate referrals; develop interim case plan and work with participants to ensure goals are met.
• Introduce and educate families on department services and community resources, the assessment process, and participant rights.
• Assist and support families in the identification of needs, completion of applications, development of service goals, and the assessment process.
• Refer cases to cross-program teams for development of an integrated service plan and ongoing case management.

MINIMUM QUALIFICATIONS:

You must possess all the minimum qualifications below to pass the exam for this position. • Good knowledge of human services principles and practices . This is typically gained by a bachelor’s degree or higher in a human services field (social work, education, nursing, psychology, counseling, sociology, etc.) OR at least three years of work experience as a human services professional (e.g. social worker, counselor, rehabilitation specialist, etc.) assisting individuals and families with accessing community resources including benefits and services.
• Experience in case management . This is t ypically gained by at least one year of professional case management experience that included case plan development and monitoring.
