The Resource Coordinator (Care Manager) is responsible for participating in outreach and enrollment, and providing assistance/guidance to individuals, family members, and caregivers on available services. The Resource Coordinator (Care Manager) serves on an inter-disciplinary team that provides service coordination, managing the medical, social, and behavioral health needs of enrolled members. This role oversees and facilitates access to all necessary services for individuals/families to stay healthy, prevent hospitalizations, and maintain stability.

2 openings - both require home visits (Queens or Long Island area)

Essential Functions:

  • Conduct recruitment efforts/activities and actively monitor waiting lists of eligible individuals/families to ensure accurate placement.
  • Complete intake, enrollment, and ongoing assessments.
  • Facilitate interdisciplinary collaboration among all providers, enrollees, families, caregivers, and available supports.
  • Ensure families are aware of all support services and entitlements. Provide recommendations and referrals to relevant community resources. Advocate for individuals/families as necessary by serving as a liaison between individuals/parents and other programs.
  • Conduct office/home/community visits to assess family needs and service delivery.
  • Perform outreach to program candidates among client populations within the community.
  • Assist in coordinating services for special needs, providing necessary support to families with children with disabilities.
  • Monitor and maintain compliance with all Federal, State, and Local regulations.
  • Monitor compliance using data management systems.

Minimum Qualifications:

  • Bachelor’s degree in Human Services or a related field.
  • 2 years of experience working in Human Services, Psychology, or a related field that included coursework in the principles of social work, child development, counseling, or psychology.
  • Ability to plan and carry out assignments independently.
  • Ability to prioritize, adhere to timelines, and multi-task.
