Resource Care Navigator (BSW)

Job Summary - Position Overview

The Resource Care Navigator plays a critical role in the continuum of services for individuals and families who receive medical care at FHCSD. The Resource Care Navigator will operate out of the entrance/waiting room areas of our ambulatory care clinics, welcoming and greeting individuals, families and guests. The Resource Care Navigator will help create an environment of a warm hospitality atmosphere that is caring yet efficient. The Resource Care Navigator will also work in the capacity of being a resource expert in SDOH, helping patients complete SDOH screening questions during registration and individualized resource and education based on their individual SDHOH screening needs. The Resource Care Navigator will serve as an ambassador of FHCSD helping them navigate care providing ongoing care that is client-centered and outcome-oriented.

The Resource Care Navigator will stay informed on community resources, serve as an expert of Social Determinants of Health and provide resource information for individuals and families who need assistance whether its resource navigation, mental health navigation, direct financial assistance, food and transportation to individuals of all ages that are struggling to maintain socio-economic emotional stability. The Resource Care Navigator will also educate internal stakeholders.

Essential Functions

Greeter

  • Cordially greets visitors and patients upon arrival. Ascertains patients’ and or visitors’ needs and directs them and or assist them through a warm hand off, accordingly.
  • Provides general information about FHCSD and assists with functions like assistance with pre-registration, completion of forms, kiosk matters, etc.
  • Monitors the overall appearance of the ambulatory care/clinic entrance.
  • Performs related duties as required.

Care Navigation

  • Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards. Assists in preparing reports as required.
  • Assists patients to complete SDOH Self screening.
  • Review patient SDOH self-screening tool results to individual patients.
  • Identify SDOH barriers to care and prepare a tailored list of resources to meet the identified needs prior to patient leaving the clinic.
  • Maintain client information and case documentation in a confidential and professional manner in EHR with integrity and in line with the quality standard set by supervisor.
  • Keep up with knowledge of social service San Diego County.
  • Consult and coordinate with FHCSD social services department, partners to establish warm hand-offs and referrals.
  • Attend to telephone calls, assessing each situation and screening for high-risk situations.
  • Attend program related and educational meetings.

Requirements:

  • BSW or bachelor's degree in human services field, or related field preferred.
  • 1 year of work experience providing human services to high risk, medically underserved, disabled populations, and/or relevant community health populations required.
  • Minimum of 6 months of experience in a case management/resource coordination role.
  • Knowledge of Social Determinants of Health.
  • Knowledge of resources available in San Diego County.
  • Demonstrated persistence in finding resources that address the multi-factored impacts of poverty.
  • Ability to convey positive, warmth, empathy, and hospitality.
  • Ability to work with multiple software systems, including case management database.
  • Comfort in providing services to clients in person and in community, as needed.
  • Bilingual in English/Spanish may be required depending on assignment.
  • Demonstrated ability to be culturally sensitive and respect diversity.
  • Excellent interpersonal and customer service skills.
  • Excellent written and verbal communication skills.
  • Knowledge of the community resources, health and social service systems in San Diego County and skill in establishing working relationships with community partners

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors – we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
