Responsibilities

  • Adheres to personnel policies and agency requirements for ethical and professional behavior.
  • Protects client's confidentiality and personal rights.
  • Keeps clear, concise and accurate records of clinical activity.
  • Adheres to 15-minute checks for inpatient clients and 60-minute checks for SAR/Woodland.
  • Participate in staff meetings and coordinate activities with program staff.
  • Provide assertive outreach to clients with engagement challenges.
  • Establish and maintain relationships with personnel and referral resources.
  • Coordinate and accompany clients to Telemedicine visits and maintain records.
  • Provide services per program requirements and individual treatment plans, including living skills and activities.

Lines of Authority

This position is supervised by the Inpatient Administrator, Program Manager or his/her designee. Unless specifically designated this position does not supervise other personnel.


Position Summary

Provide activities as assigned based on specific site and population served (Inpatient and Outpatient facilities) assisting with daily living skills and care working towards independence. The BHPP is expected to cooperate as a team member in the provision of a therapeutic environment that encourages each client's emotional growth. The BHPP is required to actively participate with the client in a positive manner. This position will be expected to maintain an acceptable level of productivity as measured by billable hours. Implements policy on infection control, kitchen usage and housekeeping. Advocates for clients, obtain entitlements, and coordinate transportation between ChangePoint and other service providers. Report any unusual behavior or medical issues to the supervisor and or nurse.


Duties and Responsibilities

1.Adheres to personnel policies and comply with agency requirements for ethical and professional behavior.


2.Protects client's confidentiality and personal rights.


3.Keeps clear, concise and accurate records of clinical activity according to Policy and Procedures.


4.Adheres to completion and documentation of 15-minute checks on Psych Inpatient clients, and 60-minute checks on SAR and Woodland clients.


5.Participate in staff meetings. Coordinate activities with other program staff.


6.Provides assertive outreach to clients who resist involvement or have difficulty accessing services.


7.Establishes and maintains a working relationship with personnel and referral resources.


8.Coordinates and accompanies Clients to Telemedicine provider visits and completes record keeping requirements for visits.


9.Provide services and activities for clients in accordance with program requirements and individual treatment plans. Such services and activities may include:



  • Serve as primary care coordinator for assigned clients; provide educational and living skills discussions

  • and activities, (e.g., laundry, cooking, cleaning, shopping, personal hygiene, Etc.) based onsite/population;

  • Participation in and development of group trainings for clients

  • Attend staffing and medical appointments,

  • Foster the development of independent living skills

  • Monitor client behavior, foster and model socially acceptable behavior patterns;

  • Provide recreation and socialization;

  • Provide transportation to appointments and approved activities


Primary Contacts

Frequent interaction with customers and their support system usually their family.


Interface as needed with peers and other clinical team members.


Collaboration as appropriate, with other community resources and referral agencies, with the client's consent.


Position Requisites

Qualifications and Experience

Required:


High School diploma or GED


Current Arizona Driver's License.


CPR and First Aid certification.


Comply with a Drug Free workplace.


Knowledge and ability to define the needs of the customer served.


Basic computer skills


Preferred:


One year of experience working with individuals in a behavioral health setting, or a combination of education and experience


Knowledge of entitlement process and local resources.


Skills/Abilities

Required:


Fluent in English both verbally and in writing.


Able to prioritize workload.


Able to work in crisis-oriented work environment and to effectively employ analytical and problem-solving skills.


Ability to prioritize, handle multiple priorities on the unit.


Ability to monitor and use Universal Precautions for Infection Control.


Ability to supervise and perform Seclusion/Restraint procedures.


Ability to complete required reports and/or paperwork.


Preferred:


Experience and training in crisis intervention


Familiar with psychotropic drugs and their side effects.


Working Conditions

Physical Requirements

Able to sit and stand for extended periods of time, less frequently,


walking, lifting and reaching.


Able to hear ordinary conversation and phone communications.


Visual capacity necessary to fulfill documentation, responsibilities of job.


Equipment Operation

Able to operate general office equipment.


Able to operate Company vehicles.


Environmental Conditions

Air conditioned and/or heated office setting.
