Job summary

You will help with daily living activities, community participation, and personal goal attainment while ensuring a safe, supportive environment. The role emphasizes advocacy, inclusion, and building meaningful relationships.

Qualifications

• High School Diploma, GED or equivalent is required.
• Valid Driver’s License and acceptable driving record are required.
• Reliability, willingness to learn, and openness to new opportunities are important.

Responsibilities

• Develop individualized, person-centered service and support plans and help implement them.
• Provide support with daily living activities, skill development, and personal goal achievement.
• Assist individuals with transportation and accessing community resources as needed.
• Promote community inclusion, relationship building, personal growth, and valued social roles.
• Maintain a safe, healthy, and supportive environment for individuals served.
• Complete and maintain required documentation in a timely, accurate manner.
