Responsibilities

• Assist in developing individualized service and support plans and help implement them.
• Provide daily living support, skill development, and help clients achieve personal goals.
• Assist individuals with transportation and accessing community resources as needed.
• Promote community inclusion, relationship building, personal growth, and valued social roles.
• Maintain a safe, clean, and supportive environment and complete required documentation timely.
