PRIMARY PURPOSE: to improve the quality of life, independence, and health of the individuals living in the assigned home.

LOCATION: BRADFORD, MA

SCHEDULE: 28 Hours, Sat & Sun 9a-11p

Responsibilities:

• Complete Individual Support Plan (ISP) goals and activities. Assist each individual in developing skills in community living, including self-care, household tasks, and socialization.
• Ensure safety and assist with personal care activities.
• Administer medication when assigned.
• Complete daily logs and notes, track behavioral data, and record medication, seizures, illnesses, incidents, and activities related to behavior plans.
• Assist individuals to participate in community activities, seek membership in community groups, and build personal relationships.

Other Duties:

• Coverage: Responsible for residence coverage during hours specified by the Program Director.
• Money management: Maintain personal budgets for residents, plan expenses, and include residents in budgeting.
• Behavior: Guide individuals to behave appropriately per behavior plans.
• Nutrition: Provide meals with attention to nutrition, variety, individual choice, and diets.
• Home management: Assist with household tasks and report repairs.
• Attend regularly and predictably.
• Other duties as assigned by the Residential Program Manager.

Qualifications:

• High school diploma or GED.
• Reading, Writing, and Math skills at high school level.
• Computer literacy.
• Medication Administration Program (MAP) certification preferred or certifiable.
• CPR and First Aid certification.
• Valid driver’s license, good standing, and at least one year of driving experience.
• Experience with adults with intellectual and developmental disabilities.
• Regular attendance.
